Abstract/Notes Objective: This technical report describes a “hybrid horizontal” (HH) radiograph of the upper cervical spine.

Methods: The base posterior and vertex are traditional views used in upper cervical specific (UCS) chiropractic to view the atlas in a horizontal plane. Where both require a tilting bucky, a HH projection does not. The patient faces a vertical bucky with the chin elevated. The x-ray tube is set to an SID of 100cm; a caudal angle of 30o; and aimed through the vertex of the head and the tragus of the ear.

Discussion: The author has used this view in lieu of a vertex projection in a UCS practice for one year with positive results and without any noticeable detriment in clinical efficacy.

Conclusion: The hybrid horizontal projection may be an acceptable alternative for UCS radiographic analysis.
